What is the case manager's role in meeting the health and welfare assurance, cont'd?


  • Maintain regular communication with participants and providers during the initial and ongoing service plan implementation. Be alert to early warning signs which, left unaddressed, could potentially cause harm. Document your observations and actions so that others are also alert to the situation.
     
  • Inform participants of their right to be safe, how to keep safe, and how to report problems.  In the case of vulnerable participants reluctant or unable to speak for themselves, use discretion in deciding whom to involve in these conversations.

  • Help implement a strategy for effectively addressing the situation and monitor how well it is working. 

  • Contribute to quality improvement by periodically helping to develop practices and approaches for systematically reducing the incidence of abuse, neglect and exploitation within your agency. 
Your role under this assurance is one of astute observation, documentation and action. Failure to meet the intent of this assurance – keeping participants safe – brings serious consequences. While all risks cannot be avoided, your role is to anticipate, rapidly report and effectively respond when they do occur.
